Art Class Application Form

Registration form, responsibility waiver, and a USD $200.00 deposit are required in order to reserve a place in the art class of your choice. You will receive confirmation once this has been done.

Application can be completed in 3 easy steps.

  1. Completely fill out ALL * marked areas of the registration form and click on the link that says 'submit' to submit the registration form. (Once received by Neva Bergemann and Artists On The Move, you will be sent information on how to proceed for steps two and three.)
  2. Read, sign and send the responsibility waiver.
  3. Send the deposit or payment in full, in U.S. dollars for the workshop of your choice via bank wire transfer or Internet Bill Pay, if you have this option in place with your bank.
